Please read, complete & sign Pages 5 & 6. As per additional “Required documentation” areas highlighted in red on Page 6 please ensure you have attached the required documents from your fund.

WHS Workstation Self-Assessment
Your safety is important to us. We will conduct onsite checks to ensure you are in a safe environment but recommend you also take the attached workstation self-assessment. Should the role you were originally hired for significantly change or should you have any concerns for your safety or wellbeing in the workplace then we require you to contact Spring Professional immediately.
